1352 Client presented to Urgent Care with bandage on left forearm. Client states “it hurts, but my pride is more wounded.” Client escorted to room 12. 1358 Bandage removed by this RN. Several clots noted on bandage and wound. Wound has minimal blood seepage and measures 13 cm long x 2 cm wide x 1.5 cm deep. No odor noted. Wound has numerous particles of dirt, leaf, and wood debris. Wound flushed at this time with normal saline.
